Department Summary
UNT Health, Clinical Practice Group is the academic clinical practice for the Health Science Center. Focused on improving health outcomes to build a healthier community. The UNT Health primary and multispecialty care clinics are located on the HSC campus in Fort Worth. Our care teams extend to local hospitals, surgery centers, long-term and continuing care centers, and mobile and in-home care. The Clinical Practice Group’s interprofessional teams work collaboratively to achieve an optimized treatment plan tailored to the individual patient. Physicians, nurse practitioners, physician assistants, physical therapists, pharmacists, social workers, psychologists, specialized technicians, and nurses work together while intentionally integrating health care students, residents, and fellows to deliver care to our community.
Position Overview
The Licensed Social Worker will conduct comprehensive intake and assessments to implement and develop individualized service care plans and provide case management in a primary care clinic setting.
Minimum Qualifications
Bachelor's degree in Social Work or related field and one year related experience; or current and unrestricted license (LBSW) to practice Social Work in the State of Texas.

- Ability to communicate effectively with a wide cross section of individuals.
- Clinical social work practices and modalities.
- Knowledge of medical environments and protocols.
- Ability to think critically.

Required License/Registration/Certifications
Requires Bachelor's degree in Social Work with current and unrestricted license to practice Social Work in the State of Texas.
Job Duties
- Engages client by maintaining regular contact to monitor and track progress and response to services.
- Develops and implements individualized service plans with the client to address needs within the established time frame.
- Performs comprehensive assessments on clients according to established standards.
- Advocates for clients at service providers or agencies and in the community.
- Assist medical and clinical staff with the preparation, processing, and completion of required care forms and clinical documentation.
- Facilitate patient referral pipelines by coordinating incoming requests and managing necessary insurance documentation.
- Support the care team in executing and documenting risk mitigation protocols and patient safety interventions.
